Chilcote Surgery Telephone Number

Access on-line Services

Patients may now order repeat medication, book GP telephone calls, see their immunisation record and submit change of address details on-line. Access to additional information in patient records is also available. Please ask if you would like this in addition.

There are two ways to register for online services. You can either:

  • Follow the Patient Access logo below and follow the steps to register for an account. You will be able to book one GP phonecall once registering then you will need to come in to show two forms of ID to fully access your account.

  • Come into the surgery, bring with you some photo identification, collect a registration form, and register online.  You will then be able to use the online system.

Please note that we do not register children under 16 currently but are working with our clinical software provider to enable this in the future. However, a registered parent may book a GP telephone call on behalf of a child by adding the child’s name in the “reason for appointment” section.

Click on the following link to learn more about the system and to log in:

You must bring with you some photo identification such as a photo driving licence or passport before you can register.  Photo ID is necessary for security reasons to ensure that access to your records and medication is only issued to you.

You must register in person in order to use this facility.

From 17 June 2013 the GPs will book all their own appointments and you will be able to book a GP telephone call on-line.  If the telephone call needs to be at a specific time please enter this information in the notes section on the 'Confirm your Booking' screen.

N.B.  Please note that the time slot you select for a GP telephone call will NOT reflect the time you will be telephoned.  GP calls will be made from 8 am onwards or possibly a little earlier than this.
